The Na Homolce Hospital Department of Vascular Surgery was founded in 1990. During its existence its profile has been that of a national and international center, admitting patients from throughout the Czech Republic and abroad. It is the largest specialized vascular surgery center in the Czech Republic and acts as a training center for Vascular Studies at the Institute of Health Care Post-Graduate Education. It also functions as a consultancy for acute and complicated angiosurgical cases. Over 70 percent of the department's capacity is devoted to operations categorized as acute vascular reconstruction surgery.
We perform complex diagnostic and surgical interventions on diseased arteries and veins, particularly when these have become constricted or occluded as a result of atherosclerotic changes, and on injuries to the vascular system outside the area of the coronary arteries, the ascending aorta and aortal arch, which have traditionally been the scope of cardiac surgeons. The range of surgical interventions include operations on branches of the aortal arch, operations on the abdominal aorta including treatment of aneurysms, reconstruction of the pelvic arteries and cardiac valve surgery. One of the largest group of patients we deal with suffer from restricted blood flow to the lower limbs and inadequate cerebral blood supply due to constriction of the arteries supplying blood to the brain. The minimally invasive interventions performed by the department include thoracoscopic sternal sympathectomy and endoscopic operations on arteriovenous fistulas. Since 2002 we have also been performing surgery on the abdominal aorta using minilaparotomic methods, which contributes to a significant reduction in the size of the surgical wound. We are the only facility in the Czech Republic to perform certain interventions in the aortoiliac area laparoscopically, a method we have been using since 2003.
Our outpatient clinic registers patients and ensures their initial physical and laboratory examinations, basic instrumental screening and any more complex planned examinations. Patients may be referred to our outpatient department by their GP or a specialist, but they may also arrive without outside recommendation. Angiographic screening is performed by the Department of Radiodiagnostics during a one-day ward stay. Most of the other examinations, such as CT, MR, PET etc., can be carried out on an outpatient basis; for patients living outside Prague or those who have restricted mobility, we group a number of examinations together and they are admitted to the wards for a short diagnostic stay. During this diagnostic stay, or together with other outpatient examinations, we also perform an internal pre-operative examination. After assessing the results of all your tests an indication is made, which means a decision on further treatment. Depending on the type of suitable surgery and the urgency of the treatment, patients are booked into the surgical timetable and the department secretary ensures that they are given their operation date sufficiently ahead of time. If surgery is not indicated we will recommend suitable conservative medicamentous follow-up treatment either in our hospital wards or in a surgical or internal medical facility in your area.
